Opioid Health Homes (OHH)
The Opioid Health Home (OHH) is not a residential program. It is for people with an Opioid Use Disorder (OUD) to help make sure they get the services they need. If you choose to use an Opioid Health Home, you will work with a health care team to coordinate, support, and help manage your treatment, recovery and other health care and social needs.
OHH is there to improve care coordination for physical health, Medication Assisted Treatment (for example: suboxone, methadone, etc.) and other needs, including transportation, housing, etc. With this extra support, including working with peers in recovery, you can connect to medical services and other health supports.
How do you qualify for Opioid Health Homes (OHH)?

Be a resident of Michigan; specifically, Lenawee, Livingston, Monroe, or Washtenaw Counties if you’d like to sign up with one of the providers below.
